Our approach to Life Care Management.
We have a singular focus: to help your clients live the most independent life possible. Fulfilling this is different for every individual. Our process begins with a Needs Assessment. We take into account an individual’s physical and cognitive abilities, their financial situation and, most importantly, their wishes and desires. Based on this assessment, we create a Life Map, filled with specific recommendations to address the individual’s short-term and long-term needs.
While service coordination typically includes coordinating healthcare between providers, Life Care Managers look beyond a client’s health needs and coordinate all the services that help an individual maintain a high quality of life. Finally, we provide routine on-site quality assurance.
